Hello. You should take a look at the item_template: ScalingStatDistribution (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-ScalingStatDistribution) table.
You can also take a look at how some of the already existent heirlooms has been done in the DB, I will use Tattered Dreadmist Robe as an example:
While not being different from most items, in most of the columns it do have some item specific values:
Flags (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-Flags): 134217728 which is bind to account(this flag needs the column Quality (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-Quality) to be set to 7).
While most items has their stats set in the columns stat_type (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-stat_type) and stat_value (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-stat_value), this does not.
It does however use the columns ScalingStatDistribution (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-ScalingStatDistribution) and ScalingStatValue (http://collab.kpsn.org/display/tc/Item+template+tc2#Itemtemplatetc2-ScalingStatValue) set.
You will need to play around with the values in these two, as I do not know where to find the values for this.

Regards, DarkAngel.

Edit: I do not think you can add custom values in the ScalingStatDistribution and ScalingStatValue columns: You could just any heirloom you want and then change the display id, tho this would not change maxlevel or the stats for the stat distribution... Which renders this entire post useless, sorry.

Edit 2: I was wrong, take a look at [Link Removed] Thread and Second post.
Oh and custom scaling stats require DBC editing...
